Student Health Requirements

School Physicals

Illinois law requires all students entering early childhood, kindergarten, sixth grade, and ninth grade are required to have a current physical examination completed on the approved Illinois Department of Public Health Form. Please note that a sports physical does not satisfy this requirement. 

For the 2025-26 school year, we can accept a physical dated August 21, 2024, or after to satisfy the requirement.

Eye Exam

Illinois law requires that proof of an eye examination by an optometrist or physician (such as an ophthalmologist) who provides eye examinations be submitted to the school. The examination must be completed within one year prior to the first day of the school year the child enters the Illinois school system for the first time. 

For the 2025-26 school year, we can accept an eye exam dated August 21, 2024, or after to satisfy the requirement.

Dental Exams

Illinois law (Child Health Examination Code, 77 Ill. Adm. Code 665) states all children entering kindergarten, second, sixth, and ninth grades shall have a dental examination. The examination must have taken place within 18 months prior to May 15 of the school year. A licensed dentist must complete the examination and sign and date the Proof of School Dental Examination Form. If you are unable to get this required examination for your child, fill out a separate Dental Examination Waiver Form. Scroll down for all forms.

For the 2025-26 school year, we can accept a dental exam dated after November 15, 2024.

Health Requirements by Grade

Health Requirements for New-to-Illinois Student

  1. A completed physical examination on the Certificate of Child Health Examination form signed by a licensed physician or osteopath and dated no earlier than one calendar year prior to the first day of school.
    • A sports physical does not meet the requirements and is insufficient. However, the School Physical Examination form can count towards the sports physical exam requirement.
  2. Transfer students from out-of-state must provide a health form that is comparable to the State of Illinois requirements and is acceptable if the exam was done within one calendar year of entering an Illinois school. If not, the above physical exam is required within 30 days of enrollment. For students entering from out-of-country, an updated physical examination form must be completed and on file before they can attend school.
  3. Parents are to sign and complete the top medical history section of the physical form.
  4. For incoming kindergarten students, a statement from a physician or health care provider stating that the student has been screened for lead poisoning.
  5. A complete immunization record signed by the child’s health provider including:
    • DPT series with last immunization after the fourth birthday
    • Tdap vaccination for students over 11 years old and entering sixth through twelfth grade
    • Polio series with last immunization after the fourth birthday
    • Hepatitis B series (3)
    • Measles, Mumps and Rubella
    • Meningococcal for sixth grade and twelfth grade
    • Varicella (2)
  6. TB Screening is recommended but not required at this time.

Vision Requirements for Kindergarten and New-to-Illinois Students for the First Time

A completed Vision Examination signed and dated by an Optometrist or medical doctor who performs eye examinations is due by October 15 of the current school year. A completed Vision Waiver may be submitted if your student is enrolled in AllKids/Medicaid and you are unable to find a provider; you do not have vision insurance including AllKids/Medicaid and there are no low-cost clinics in the community; or any undue burden or lack of access to a provider of eye examinations.

Certificate of Religious Exemption

Families who object on religious grounds to complete any required immunizations or examinations must complete a Certificate of Religious Exemption form, which includes a statement of their religious objections. The form must also be signed by a healthcare provider. The healthcare provider signature on this new form attests to informing the family of the benefits of immunization and the health risks associated with not vaccinating the student. Per the State of Illinois, the certificate requires the signature of a parent or guardian to attest to understanding that their child may be excluded from school in the case of a vaccine-preventable disease outbreak or exposure.
